Please review the Chromebook Agreement Contract with your BCHS student. Once you have, complete the form below to give your student permission to check out a school Chromebook. If any student has outstanding fees or a current Chromebook checked out in their name, they will not be assigned a new one until fees are paid and/or outstanding device is returned. 

You must fill out a separate form for each of your BCHS students. Completing this form acts as an electronic signature.
